Job summary

Paycom - ATS in San Antonio, TX is seeking a Certified Nursing Assistant (CNA) for the 10:00 pm to 6:00 am shift in a Skilled Nursing setting at Sorrento.

You will provide direct patient care, assist with ADLs, document care, and follow individualized care plans to ensure safety and comfort for residents. An active CNA certification is required and the role includes adapting to lift and move patients safely.

Qualifications

• Deliver direct patient care including bathing, dressing, grooming, toileting, and feeding.
• Assist with patient transfers, ambulation, and repositioning using safe lifting techniques.
• Monitor, observe, and report changes in patient condition to the Charge Nurse.
• Accurately document patient care and daily activities in accordance with regulations.
• Support and follow individualized care plans to improve patient outcomes.

Responsibilities

• Maintain a clean, safe, and infection-controlled environment.
• Ensure medical equipment and devices are clean, charged, and functioning properly.
• Participate in shift handoffs, team communication, and ongoing training.
• Follow all state, federal, and facility policies and procedures.
