Job summary

Cantex Continuing Care Network is hiring a Certified Nursing Assistant (CNA) at Solera at West Houston in Houston, TX. You will provide direct patient care, assist with ADLs, and support a safe, dignified environment in a skilled nursing setting.

This PRN role offers flexible scheduling, teamwork within a high-performing care team, adherence to state and facility policies, and opportunities for ongoing training and CEU reimbursement.

Qualifications

• Active CNA certification required.
• Ability to lift, move, and transfer patients safely.
• Ability to read, write, and speak English.

Responsibilities

• Deliver direct patient care, including bathing, dressing, grooming, toileting, and feeding.
• Assist with patient transfers, ambulation, and repositioning using safe lifting techniques.
• Monitor, observe, and report changes in patient condition to the Charge Nurse.
• Accurately document patient care and daily activities in accordance with regulations.
• Support and follow individualized care plans to improve patient outcomes.
• Maintain a clean, safe, and infection-controlled environment.
• Ensure medical equipment and devices are clean, charged, and functioning properly.
• Participate in shift handoffs, team communication, and ongoing training.
• Follow all state, federal, and facility policies and procedures.
• Promote patient dignity, independence, and safety at all times.
