Responsibilities

• Provide routine daily care to assigned patients following their clinical assessments and care plans.
• Assist with personal care needs such as bathing, dressing, feeding, toileting, ambulation, transfers, and positioning.
• Support patients with physician-ordered medications that are self-administered.
• Ensure patient safety and comfort at all times.
• Monitor and report on patients’ health conditions promptly.

Current Available shifts include:1. Full Time PM shift (3pm - 11pm) on Skilled Nursing Post Acute2. Full Time Noc Shift (11pm - 7AM) on our Special Treatment Program (Behavioral Health)
