Job Responsibilities

• Provide best practice nursing care to post-acute residents in the area
• Monitor residents' conditions and update the nursing team
• Record vital signs accurately
• Assist residents with grooming, feeding, toileting, and ADLs
• Maintain hygiene and safety standards
• Transport residents and assist with mobility
• Engage with residents to provide comfort and build trust
• Follow care plans and comply with regulations
• Communicate effectively with staff, residents, and families

Qualifications and Skills
