Nature Trail Health & Rehab Center in Mount Vernon, IL is hiring Certified Nursing Assistants (CNAs) to join our compassionate care team and support residents across all shifts. In this role you will assist with meals, hydration, bathing, grooming, and mobility; monitor residents’ condition and promptly communicate changes to the nursing staff.

• Current Certified Nursing Assistant (CNA) certification in Illinois.
• Graduate of an accredited CNA training program.
• Reliable transportation.
• Strong teamwork, communication, and resident-focused care skills.
• Consistent and dependable attendance history.

Responsibilities

• Assist residents with meals, hydration, and feeding support.
• Make routine rounds and respond promptly to resident needs.
• Reposition residents to prevent discomfort or skin breakdown.
• Monitor vital signs and report changes to nursing staff.
• Assist residents with bathing, grooming, and personal care.
• Provide proper skin care and report observations of breakdown or pressure areas.
• Maintain reliable attendance and punctuality.
• Complete additional duties as assigned by leadership.
