Job summary

Wellspring Lutheran Services in Monroe, MI is seeking a CNA to provide direct resident care, assist with ADLs, and follow individualized care plans. You will report changes to nursing staff while preserving dignity and comfort, and support residents’ rights with prompt reporting of any concerns.

The role offers full-time or part-time hours on the Afternoon Shift, with a $2.00/hour shift differential, a $1,500 sign-on bonus for full-time, and comprehensive benefits including health and retirement

Qualifications

• High school diploma or equivalent.
• Current Certified Nursing Assistant license in Michigan.
• Minimum of 6 months CNA experience in long-term care or skilled nursing preferred.
• Strong interpersonal and communication skills with compassionate care.

Responsibilities

• Provide direct resident care with ADLs (bathing, grooming, feeding, toileting, ambulation).
• Follow care plans and report changes in residents’ conditions to nursing staff.
• Respect residents’ dignity, privacy, and comfort in all care actions.
• Support residents’ rights and report abuse, neglect, or restraints immediately.
• Protect residents’ belongings and help locate lost items.
• Promote skin integrity, turn/reposition to prevent ulcers, apply skin care as directed.
• Respond promptly to call lights and resident needs.
• Assist with vital signs, weight checks, specimen collection, and creams/ointments.
• Complete CNA documentation accurately per policy.
• Follow infection control and safety procedures (hand hygiene, gloves, disposal).
• Welcome and orient new residents and families to the facility.

Location: Wellspring Skilled Nursing & Rehab - Monroe

Reports to: CNA Supervisor / Director of Nursing (DON)

Position Type: Full-time or Part-time | Hourly

Responsibilities

• Provide direct resident care, including assistance with Activities of Daily Living (ADLs) such as bathing, grooming, oral hygiene, feeding, toileting, colostomy care, ambulation, transferring, and range-of-motion support.
• Follow individualized care plans, reporting observations and changes in residents’ conditions to nursing staff.
• Ensure residents’ dignity, privacy, and comfort are prioritized in all aspects of care.
• Support residents’ rights, reporting any concerns of abuse, neglect, or restraint use immediately.
• Protect residents’ personal belongings and assist in locating lost items.
• Promote skin integrity by preventing pressure ulcers, turning and repositioning residents, and applying skin care as directed.
• Respond promptly and courteously to call lights and resident needs.
• Assist with vital signs, weight checks, specimen collection, and application of creams/ointments.
• Accurately complete CNA documentation according to policy.
• Follow infection control and safety procedures, including proper hand hygiene, glove use, and disposal of soiled materials.
• Welcome and orient new residents and families to the facility.

Schedule And Compensation

• Employment Type: Full-time or Part-time
• Schedule: Afternoon Shift (1:45 pm - 10:15 pm)
• Weekends: Every other weekend rotation required
• Pay Rate: $21.40–$24.40/hour based on licensure
• Shift differentials: $2.00 per hour for Afternoon Shift
• Sign-on Bonus: $1,500 sign-on bonus available for Full-time
