Qualifications

Responsibilities

• Provide routine daily nursing care to residents according to each resident's care plan.
• Assist with activities of daily living including grooming, bathing, dressing, and toileting.
• Help residents with mobility, positioning, and restorative activities.
• Support dining assistance and nutritional needs.
• Assist with oral care, hygiene, and skincare.
• Follow infection prevention protocols and maintain clean environment.
• Report changes in residents' condition promptly to nurses.
• Document all care accurately and maintain up-to-date records.
• Collaborate with interdisciplinary team to ensure high-quality resident care.

• Resident centered care oriented including assistance with activities of daily living such as:


• Positioning, Moving, and Restorative Care.


• Mobility, Toileting, and Bathing.


• Grooming: Oral, Shaving and Nail Care.


• Dining Assistance.


• Dressing and Undressing.


• Infection control and prevention.


• Nutrition and Elimination.


• Comfort care and End of Life Care.


• Answer call lights timely and promptly.


• Report all changes in the resident's condition to your assigned nurse as soon as possible.


• Report all accidents and incidents you observe on the shift when they occur.


• Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.


• Be a team player working in coordination with all disciplines in the facility.
