Job summary

Evergreen Healthcare Group's Hillsboro Health and Rehabilitation Center in Hillsboro, OR is seeking a Certified Nursing Assistant (CNA) for weekend, part-time shifts. You will provide direct resident care, assist with ADLs, meals, and mobility.

Responsibilities

• Answer signal lights.
• Assist with bathing, dressing, toiletry and oral hygiene activities of daily living (ADLs).
• Serve and feed residents requiring help.
• Transport residents, using wheelchair or wheeled cart, or assist residents to walk.
• Turn and reposition residents, alone or with assistance.
• Take and record vital signs, intake/output, and weight, as directed.
• Complete documentation.

Job Summary

Hillsboro Health and Rehabilitation Center is looking for Certified Nursing Assistant (CNA) to perform a combination of duties in caring for residents in the center, consistent with the plan of care and established long-term care standards.

Essential Functions:

• Answers signal lights
• Provides assistance with bathing, dressing, toiletry, and oral hygiene activities of daily living (ADLs).
• Serves and collects food trays and feeds residents requiring help.
• Transports residents, using wheelchair or wheeled cart, or assists residents to walk.
• Turns and re-positions residents, alone or with assistance
• Takes and records vital signs, food and fluid intake and output, and weight, as directed.
• Completes documentation

Qualifications:

• Required: Current certification as a nursing assistant in the state of Employment
• Required: High school graduate or equivalent
• Ability to work well in a team setting
