Our CNAs play a vital role by providing essential, hands-on care and meaningful support. We are seeking CNAs in Conroe, TX, and surrounding areas.

About the Role

As a Home Health CNA with Pathfinder, you will:

1. Provide personal care and daily living assistance under the guidance of a Registered Nurse and in accordance with each patient’s care plan.
2. Work one-on-one with patients in their homes, helping them maintain independence, dignity, and comfort.
3. Be supported by a collaborative clinical team.

Essential Job Functions

• Provide hands-on personal care, including bathing, grooming, toileting, dressing, and feeding.
• Maintain patient comfort and dignity, supporting physical and emotional needs during end-of-life care.
• Accurately document all care and services provided, ensuring compliance with policies and standards.
• Communicate changes in patient condition promptly to the nurse case manager.
• Operate and assist with durable medical equipment safely.
• Monitor and report safety concerns or unusual observations.
• Collaborate with the hospice team to ensure continuity of care.
• Provide companionship and a calming presence to patients and families.

• Active Texas CNA license
• Current CPR certification
• Valid driver’s license, reliable vehicle, auto insurance
• Able to lift up to 50 lbs
• Clear background check
