About the Role
As a Home Health CNA with Pathfinder, you will:
- Provide personal care and daily living assistance under the guidance of a Registered Nurse and in accordance with each patient’s care plan.
- Work one-on-one with patients in their homes, helping them maintain independence, dignity, and comfort.
- Be supported by a collaborative clinical team.
Essential Job Functions
- Provide hands-on personal care, including bathing, grooming, toileting, dressing, and feeding.
- Maintain patient comfort and dignity, supporting physical and emotional needs during end-of-life care.
- Accurately document all care and services provided, ensuring compliance with policies and standards.
- Communicate changes in patient condition promptly to the nurse case manager.
- Operate and assist with durable medical equipment safely.
- Monitor and report safety concerns or unusual observations.
- Collaborate with the hospice team to ensure continuity of care.
- Provide companionship and a calming presence to patients and families.

Requirements
- Must be 18+ years old
- Active Texas CNA license
- Current CPR certification
- Valid driver’s license, reliable vehicle, auto insurance
- Able to lift up to 50 lbs
- Clear background check
- Home Health or Hospice experience preferred
- Strong communication and independent work skills
