Job Description

Summary of Essential Functions: A Home Health Aide (HHA) PRN, committed to caring, is trained and competent to perform hands-on personal care, simple procedures as an extension of therapy or nursing services, assistance in ambulation exercises, and administering medications (limited to assisting with water or fluids) under the supervision of a Registered Nurse (RN). The role involves contributing to the QAPI program and participating in interdisciplinary patient assessments. The HHA collaborates with patients, their representatives, and caregivers to develop and revise care plans.

Qualifications
  • Meets one of the requirements of the Certified Nursing Assistant (CNA) in the state(s) of practice, preferred.
  • CPR certification required.
  • Ability to exercise initiative and independent judgment.
  • Strong communication skills, both oral and written, with excellent interpersonal skills.
  • Good customer relations skills and a commitment to quality service.
  • Supportive of the core values and goals of the agency.
  • Valid driver’s license, auto liability insurance, and reliable transportation; 100% travel may be required.
Educational Requirements
  • High School diploma or GED.
  • Completion of at least forty hours of approved training and competency course as per Medicare and state requirements.
Working Conditions & Physical Requirements
  • Work involves indoor and patient home environments, with travel between locations.
  • Requires physical effort including kneeling, squatting, reaching, twisting, climbing, walking, and assisting with patient transfers.
  • Must have sensory capabilities or adaptive devices to perform essential duties.
  • Must provide evidence of annual TB testing and other state-mandated tests.
  • Exposure to elements, odors, blood, body fluids, and hazardous materials is possible.
