Job Summary

A Home Health Aide (HHA) PRN, trained and competent, performs hands-on personal care, simple procedures as an extension of therapy or nursing services, assists with ambulation, and helps administer medications (limited to assisting with water or fluids) to patients in their homes under the supervision of a Registered Nurse (RN). The staff, whether employees or under arrangement, contribute to the QAPI program and participate in interdisciplinary assessments. The HHA collaborates on developing and revising the care plan with the patient, their representative, and caregivers.

Qualifications
  • Meets one of the requirements for practice as a Certified Nursing Assistant (CNA) in the applicable state(s), preferred
  • CPR certification required
  • Ability to exercise initiative and independent judgment
  • Excellent communication and interpersonal skills
  • Good customer relations skills and a commitment to quality service
  • Supportive of the agency's core values and goals
  • Valid driver’s license, auto liability insurance, and reliable transportation; 100% travel may be required
Educational Requirements
  • High School diploma or GED
  • Completion of at least forty hours of approved training and competency course per Medicare and state requirements for Home Health Aide training
Working Conditions & Physical Requirements
  • Work indoors in offices and patient homes, with travel involved
  • Requires physical effort including kneeling, squatting, reaching, twisting, climbing, walking, and assisting in lifting/transferring patients up to 100 pounds
  • Must have senses functional or use adaptive devices; evidence of annual TB test and other required exams
  • Exposure to elements, odors, blood, body fluids, and hazardous materials may occur
