Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center is seeking a Certified Nursing Assistant to provide direct resident care under licensed supervision in a 69-bed facility in Bardstown, KY. CNAs support daily living activities, monitor vital signs, and help maintain a compassionate environment aligned with our resident-centered care model.

You will work collaboratively with a nursing team, communicate effectively, and contribute to a culture of quality outcomes.

Qualifications

• Active Nursing Assistant certification from the state where the facility operates.
• Effective verbal and written communication skills.
• Ability to work collaboratively within a nursing team and under licensed staff guidance.

Responsibilities

• Provide activities of daily living (ADLs) within the scope of practice and resident plan of care.
• Record resident vital signs, weights, intake/output, and other data.
• Assist with turning, positioning, and transfers with proper body mechanics.
• Respond to call lights promptly and support resident rights and comfort.
• Assist in orientation and training of other staff as requested.
• Support housekeeping standards and resident environment.
• Participate in special projects and duties as assigned.

How You Will Make a Difference
Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents in accordance with facility policies and procedures.

• Provide activities of daily living (ADLs) within the scope of practice as defined by state regulation and the resident’s plan of care.
• Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis.
• Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
• Routinely turn and position residents, as necessary.
• Recognize and support resident rights and choices within the care environment.
• Maintain established housekeeping standards within assigned duty areas.
• Promptly answer call lights and assist with resident needs.
• Assist in the orientation and training of other staff, as requested.
• Other special projects and duties, as assigned.

What You Need To Make a Difference
Current, active Nursing Assistant certification from state in which the facility operates.
Effective verbal and written communication skills.
Ability to work collaboratively within a nursing team and under the direction of licensed nursing staff.
Customer service oriented with the ability to work well under pressure.
Approachable, flexible, and adaptable to change.
Function independently, and have flexibility, personal integrity, and the ability to work effectively with stakeholders and vendors.
