Summary

Provides patient care support including hygiene, nutrition, ambulation, elimination, and safety as directed by Charge Nurse per departmental guidelines. Documents appropriately in patient records based on skill level. Completes admission procedures, records vital signs, orients new residents, and ensures proper identification and admission kits. Responds promptly to call lights and resident needs. Performs rounds, turns, and repositioning as scheduled. Takes vital signs and weights accurately, reports to Charge Nurse. Applies proper slings, splints, braces, and devices as ordered. Measures intake and output, relays information during shift change, and performs treatments as instructed. Maintains supplies and equipment, and updates charge items per procedures. Completes job duties through continuing education, accepting reassignments, and communicating effectively with team members. Maintains confidentiality and provides shift reports. Performs additional duties as assigned.

Job Qualifications

• High school diploma preferred, but not required.
• Active Certified Nurse Assistant (CNA) certification in the state of employment.
• CPR certification required or willing to obtain within six months.
• Long-term care experience preferred.
• Excellent communication skills for interactions with patients, visitors, and staff.
• Ability to manage time and organize daily tasks efficiently.
• Physical ability to stand, walk for long periods, lift heavy objects, and assist with patient transfers using proper techniques.
• Ability to push and pull equipment over 40lbs with assistance when necessary.
• Manual dexterity and coordination for working with patients and equipment.
• Ability to perform documentation accurately, either handwritten or via tape recorder.
• Visual acuity, including the ability to distinguish colors.

Pay range: $16.08 - $25.24 per hour
