Job summary

Rocky Mountain Home Health & Hospice is seeking a Certified Nursing Assistant (CNA) to deliver one‑on‑one care in clients’ homes across Carbon County, Wyoming. This community‑based role includes travel between client sites and mileage reimbursement at $0.60 per mile.

You will perform personal care tasks, monitor health status, and support clients toward independence in their own homes while maintaining dignity and comfort.

Qualifications

• Minimum requirements include CNA in good standing with the state (preferred).
• High school diploma or equivalent.
• Current CPR/BLS certification.
• Must have no disciplinary action in effect against certification.
• At least one year of CNA experience in long‑term care, home care or hospice (preferred).

Responsibilities

• Provide hands‑on personal care in clients' homes.
• Assist with bathing, grooming, dressing, toileting, and activities of daily living.
• Assist with mobility, transfers, and positioning.
• Help with meals and hydration as appropriate.
• Take and document vital signs and other observations.
• Observe changes in client condition and communicate concerns to the care team.
• Provide companionship and emotional support.
• Maintain client privacy and confidentiality.
• Complete required documentation accurately and on time.

$17.00–$18.50/hour + $0.60/mile mileage reimbursement

Rocky Mountain Care is growing our home health services in the Rawlins and Carbon County area, and we're looking for compassionate, dependable Certified Nursing Assistants (CNAs) who want to make a meaningful difference in the lives of the people they serve.

As a Home Health CNA, you won't be caring for dozens of residents in a facility. You'll have the opportunity to build one‑on‑one relationships with clients in their own homes, helping them remain comfortable, safe, and as independent as possible.

This is a community‑based role, so we're looking for CNAs who are comfortable traveling throughout Carbon County to provide care where our clients live.

What You'll Do

As a Home Health CNA, you'll provide hands‑on support that helps clients maintain their dignity, independence, and quality of life at home.

• Providing compassionate personal care in clients' homes

• Helping with meals and hydration as appropriate

Travel Throughout Carbon County Is Required

This is a home health position, which means you'll travel to clients throughout Carbon County, Wyoming.

Applicants must be willing and able to travel throughout the Carbon County service area near Rawlins.

We understand that traveling between clients is part of the job, which is why mileage is reimbursed at $0.60 per mile.

Mileage reimbursement: $0.60/mile

A reliable means of transportation and a valid driver's license are required.

Pay & Mileage

Hourly wage: $17.00–$18.50/hour

Mileage reimbursement: $0.60 per mile
