Job summary

Rocky Mountain Care is expanding its home health services in the Rawlins and Carbon County area and seeks compassionate CNAs to deliver one-on-one care in clients' homes. As a Home Health CNA, you will travel throughout Carbon County to support clients with personal care, safety, and independence.

You will document vital signs, follow care plans, and communicate with the care team while maintaining privacy and dignity for each client.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification.
• Must have no disciplinary action in effect against certification.
• At least one year of experience as a CNA in long-term care, home care or hospice (preferred, not required).

Responsibilities

• Provide hands-on personal care in clients' homes.
• Assist with bathing, grooming, dressing, toileting, and activities of daily living.
• Assist with mobility, transfers, and positioning.
• Help with meals and hydration as appropriate.
• Take and document vital signs and observations.
• Observe changes in condition and communicate to care team.
• Provide companionship and emotional support.
• Maintain client privacy and confidentiality.
• Complete required documentation accurately and on time.
