Job summary

Care 360 Hospice in Toledo, Ohio, is seeking a Hospice Certified Nursing Assistant to provide compassionate, patient-centered care and support to patients and families in home and facility settings.

The role involves travel to patient residences on a 3 PM–11 PM shift, with on-call rotation every 8–10 weeks, and requires active CNA licensure, CPR/BLS, and a willingness to travel. Hospice experience is preferred and transportation is required.

Qualifications

• Licensed Nursing Assistant (CNA) with state licensure.
• CPR/BLS Certification.
• Experience in AL/LTC/SNF or Home Health preferred.
• Must be willing to travel and have valid driver’s license.

Responsibilities

• Provide personal care per the plan (bathing, grooming, dressing, toileting, feeding).
• Travel to patients’ residences to deliver hospice services.
• Offer emotional support and companionship to patients and families.
• Provide bedside vigil support during end-of-life care.
• Assist with ambulation, transfers, positioning, and ROM per care plan.
• Safely use and assist with DME (walkers, wheelchairs, hospital beds, lifts).
• Observe and report changes to the case manager or supervising nurse.
• Document care per Hospice policies and procedures.
• Collaborate with interdisciplinary team in meetings and care planning.
• Educate family members and caregivers within the CNA scope.
• Attend on-site monthly educational meetings and training.

Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE

Schedule: Full-time | Monday - Friday

Shift Details: 3 PM - 11 PM + OC rotation every 8-10 weeks

Location: Toledo, Ohio, and surrounding areas

Requirements of the CNA role:

• Licensed Nursing Assistant in the state of practice (CNA)
• CPR/BLS Certified
• Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
• Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
• Must be willing to travel
• Valid driver's license and Auto insurance Coverage with reliable transportation
• Hospice Experience Preferred

About the Certified Nursing Assistant role:

• Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
• Travel to patients' residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.
• Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
• Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
• Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
• Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
• Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.
• Accurately document care provided in accordance with Care 360 Hospice policies and procedures.
• Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.
• Provide support and education to family members and caregivers within the scope of the CNA role.
• Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
• All other duties as assigned
