The Lakes Home Care, Inc. is hiring a compassionate Home Health Aide to assist patients in their homes per an established care plan. Duties include personal care, daily living activities, and documenting essential health information for the supervising nurse.

Requirements include CPR certification, ability to follow written instructions, and completion of Maxim competency program. Part-time position with flexible hours and a competitive salary is offered.

Formación

• Meets all training requirements per state and federal laws when applicable.
• Ability to read, follow written instructions and document care.
• Must maintain CPR certification as required by state regulation.
• Adequate health screening (PPD or chest X-ray) as applicable.

Responsabilidades

• Assist patients with daily living tasks per care plan (baths, oral hygiene, dressing, toileting, feeding).
• Record vital signs when ordered and document changes for the nurse supervisor.
• Ensure safe transfers, ambulation, and ROM exercises as prescribed.
• Support home safety and provide environmental care (laundry, safety checks).
• Observe patient condition and promptly report changes to supervising nurse.
• Communicate clearly with client, family, and office on care and concerns.
• Follow all plan-of-care directions and complete assigned tasks.

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable

• Ability to read, follow written instructions and document care given

• Possess and maintains current CPR certification in accordance with state regulation

• PPD or chest x-ray if applicable

• Successful completion of Maxim screening examinations

• Participation and completion of Maxim’s competency program
