Job summary

Wharton Nursing and Rehabilitation Center in Wharton, TX seeks a full‑time Certified Nursing Aide to provide routine daily nursing care per care plans. You will assist residents with bathing, feeding and daily activities, monitor vital signs and report any changes to the Charge Nurse.

The role requires a valid CNA license, a high school diploma or equivalent, and prior long‑term care experience is preferred. Lifting and mobility support are routine in this position.

Qualifications

Responsibilities

• Provide routine daily nursing care and services per resident plans.
• Assist residents with bathing, feeding, dressing and nutrition intake.
• Measure and record vital signs as assigned by Charge Nurse.
• Observe and report symptoms, changes or accidents to the Charge Nurse.
• Contribute to resident care plans with knowledge of residents’ needs.
• Follow resident care policy and procedures during duties.

Primary Responsibilities

The Certified Nurse Aide will provide assigned residents with routine daily nursing care and services in accordance with the resident assessments and care plans.

Essential Functions

• Assist resident in customary daily requirements and tasks in care and treatment such as bathing, feeding, dressing, observing intake of food, care of hair, nails, moving residents from area to area, responding to calls
• Carry out duties and responsibilities in accordance with resident care policy and procedure
• Measure and record vital signs as assigned by Charge Nurse
• Detect and report situations that may cause resident accidents or injuries
• Contributes knowledge of resident’s conditions/needs to resident care plans
• Observe and report symptoms, reactions, and changes of conditions to residents to Charge Nurse

Qualifications

• High school diploma or equivalent is preferred but to required

Licensing Requirements

• Must hold valid Certified Nurse Aide (CNA) license
