Bluestem PACE is seeking a full-time Home Care CNA to join our team in McPherson, KS. The role focuses on delivering compassionate in-home care, ADL support, and health monitoring for participants.

Qualifications

• Must be at least 18 years old.
• Must have current Kansas CNA Certification.
• Must have a valid driver’s license with proof of auto insurance and safe driving record.
• Must be knowledgeable of CNA principles, practices, standards and techniques.
• Must have basic knowledge in working with frail and elderly to support Activities of Daily Living.
• Must be able to establish and maintain effective working relationships with patients, co-workers, medical staff and the public.
• Must have the ability to react calmly and effectively in emergency situations.
• Must be able to work effectively with culturally, economically and educationally diverse populations.

Responsibilities

• Consult daily with Center Director or RN to be updated on participant status and care plan.
• Assist participants in the home with personal hygiene, transfers, meals, bathroom needs, and medications.

• Maintain confidentiality and charting requirements for all participants.

Position Overview

The primary responsibility of the Home Care CNA is to perform ADL's and activities within the scope of practice of a Certified Nursing Assistant such as clinical observation, clinical reporting requirements, participant laundry, participant care training and education. Assist in monitoring and reporting the health status and provide a safe environment for the participants. The Home Care CNA will be responsible to provide and assist participants with activities of daily living, restorative and supportive care. Monitor health status and provide a safe environment for participants. Function as a member of the interdisciplinary team on an as needed basis.

Essential Job Functions

• Consult each day with the Center Director or RN to be updated on participant’s status, changes in care plan and to confirm scheduled visits.
• Under the direction and supervision of the Center Director or RN and following the plan of care, assist participants in the home with the following duties:
  • Personal hygiene such as skin, mouth, hair care and bathing.
  • Transfers in and out of bed assisting with ambulation.
  • Assist with prescribed exercise as instructed by the rehab staff or nursing supervisor.
  • Prepare meals, including special diets as directed by the nursing supervisor and assist participants with meals and/or feeding when needed or as instructed.
  • Assist participants to the bathroom or in using commodes, bedpans or urinals.
  • Assist participants with medications by cueing.
  • Other duties taught by a health professional for a specific participant such as: taking vital signs, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.
• Perform directed household services.
• Report changes in participant’s condition and needs to the Center Director or RN.
• Maintain the confidentiality of all company procedures and information about participants, clients or families.
• Handle potentially infectious specimens with appropriate biohazard precautions and practice standard precautions.
• Participate as a member of the interdisciplinary team on an as needed basis.
• Assist and/or supervise participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and food nail care, and toileting) as assigned or needed under supervision.
• Assist transportation staff with the loading and unloading of participants from vehicles.
• Provide Escort assistance on transportation vehicles when requested.
• Note changes in participant’s physical or emotional health and communicate this information to the Center Director or designee.
• Maintain participant’s medical record and fulfill charting and reporting requirements.
• Assist the transportation department with participants’ takes home supplies.
• Participate in and support Quality Improvement initiatives.

• Must have basic knowledge in working with frail and elderly to support Activities of Daily Living and Incidental Activities of Daily Living
• Must be able to establish and maintain effective working relationships with patients, co-workers, medical staff and the public
• Must have the ability to react calmly and effectively in emergency situations
• Must be able to work effectively with culturally, economically and educationally diverse populations

Physical Requirements

Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
