Job summary

Bluestem Communities is seeking a full-time Day Center Certified Nurse Aide (CNA) to join Bluestem PACE in McPherson, KS. The role focuses on ADLs, restorative and supportive care, and collaborating with an interdisciplinary team to monitor health status.

Qualifications

• Must be at least eighteen (18) years of age.
• Must be Certified Nurse Aide in Kansas.
• Prefer prior experience working with the frail and elderly.

Responsibilities

• Participate as a member of the interdisciplinary team on an as needed basis.
• Carry out plan of care activities within the CNA scope of practice as directed.
• Assist and/or supervise participants with activities of daily living (bathing, dressing, grooming, mouth care, eating, toileting) as assigned.
• Serve, monitor and assist participants with meals.
• Maintain clean and orderly day rooms, kitchenette, and personal care areas.
• Accompany participants to medical appointments as needed.

Position Overview

The primary responsibility of the Day Center Certified Nurse Aide (CNA) is to perform ADL's and activities within the scope of practice of a Certified Nursing Assistant such as clinical observation, clinical reporting requirements, participant laundry, participant care training and education. It is the responsibility of the Day Center CNA to provide and assist participants with activities of daily living, restorative and supportive care. Monitor health status and provides a safe environment for participants. Function as a member of the interdisciplinary team on an as needed basis.

Essential Job Functions

• Participate as a member of the interdisciplinary team on an as needed basis.
• Carry out plan of care activities within the CNA scope of practice as directed.
• Assist and/or supervise participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and food nail care, and toileting) as assigned or needed under supervision.
• Serve, monitor and assist participants with meals.
• Throughout the day, assist in maintaining clean and orderly day rooms, kitchenette, and personal care areas.
• Accompany participants to medical and other appointments as needed.
• Perform and or assist with laundry, including clothes for participants.
• Assist transportation staff with the loading and unloading of participants from vehicles.
• Provide Escort assistance on transportation vehicles when requested.
• Note changes in participants physical or emotional health and communicate this information to the Life Enrichment Coordinator or designee.
• Maintain participant's medical record and fulfill charting and reporting requirements.
• Assist the transportation department with participants' takes home supplies.
• Act only within the scope of his or her authority to practice.
• Participate in and support Quality Improvement initiatives.
• Follow all Policies and Procedures and OSHA safety guidelines.
• Maintain the confidentiality of all company procedures, results and information about participants, clients or families.
• Handle potentially infectious specimens with appropriate biohazard precautions and practices standard precautions.
• Maintain safe working environment. Follow Safety Policies and Procedures
• Demonstrate dependability through consistent compliance with scheduled work hours.
• Participate in continuing education classes and any required staff and training meetings.
• Perform other duties as required or requested

Physical Requirements
