Qualifications

• Certified Medical Assistant with clinical experience.

Responsibilities

• Act as a community liaison between nursing, sales, wellness, and leadership.
• Coordinate care between specialists, therapy, home health, and other services.
• Assist with front desk duties, including appointment management and scheduling.
• Monthly outreach with residents/families to assess needs and coordinate care.
• Greet patients and prepare them for visits; take vital signs.
• Complete diagnostic testing and assist with QA monitoring.
• Coordinate after-visit coordination.
• Maintain a friendly, professional environment for patients and families.
