Qualifications

Responsibilities

• Act as a community liaison between nursing, sales, wellness and leadership.
• Coordinate care between specialists, therapy, home health and other services.
• Assist front desk duties, including appointment management and scheduling.
• Take vitals, greet patients, prepare them for visits and support after-visit coordination.
• Support quality assurance monitoring and clinical documentation.
