Qualifications

• Certified Medical Assistant with credential required.

Responsibilities

• Act as a community liaison between nursing, sales, wellness, and leadership.
• Coordinate care between specialists, therapy, home health, and other services; assist at front desk.
• Monthly outreach with residents/families to assess needs and coordinate care.
• Connect patients with local/regional social support services.
• Greet patients and prepare them for visits; take vital signs and room patients.
• Coordinate after-visit follow-up and complete diagnostic testing; monitor quality assurance.
