Job Overview

This position provides both direct patient care in a primary care office and works with care delivery providers to identify gaps in care, contact patients to schedule required care, and follow up on referrals. The Medical Home Medical Assistant conducts pre-visit planning, coordinates messages through electronic portals, and assists in managing transitions of care. They act as a clinical liaison to the physician care plan and communicate actively with patients. The MA participates in process improvements, understands clinical goals and outcomes, including patient satisfaction and engagement, and supports the mission, values, and management of TriHealth Physician Practices. Certification or registration as a medical assistant is required, along with strong skills in clinical care, customer service, communication, and teamwork.

This role may also be filled by an LPN or a non-certified MA with prior experience.

Location

• TriHealth Health First Physicians– Mason
• Family practice, single site - no travel required

Work Hours

• Full-time, 80 hours biweekly
• Day shift, typically 8am-5pm, hours may vary
• Weekend rotation every 4th weekend
• No holidays or on-call duties

Job Requirements

• Graduate of an approved healthcare technical, professional, or vocational program; experience in a physician practice or related field preferred
• Equivalent experience accepted in lieu of degree
• Must be certified or registered as a medical assistant and maintain certification (RMA, NCCT, NHA, or AAMA) and CPR certification
• Knowledge of medical office flow, clerical/front office tasks, EMR, practice management software, and medical coding/billing is strongly encouraged

Job Responsibilities

• Proficient in adult and pediatric rooming procedures, including clinical tasks like immunizations, venipuncture, and point-of-care testing
• Follow protocols for scheduling, procedures, and equipment use
• Accurate documentation of clinical calls, patient info, order entry, and medication management
• Respond promptly to patient messages and escalate issues as needed
• Participate in pre-visit planning, daily huddles, and care gap analysis
• Understand and communicate about wellness schedules, immunizations, screenings, and patient education
• Collaborate with other healthcare services and provide community resource information
• Support chronic disease management and wellness education
