The Neighborhood at Vestavia Hills, part of Phoenix Senior Living, seeks Med Techs to monitor residents and support wellness through medication care and care coordination.

The role includes assisting with monthly wellness visits, documenting changes, and communicating with the Wellness Director. Weekend and evening hours are required.

Candidates must be a Certified Medication Aide with at least one year of elder care experience and demonstrate leadership.

Qualifications

• Must be certified Medication Aide or meet state requirements for medication aide
• Minimum of one year of experience working with elders or disabled individuals (preferred)
• Must have demonstrated Leadership capabilities
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing (preferred)
• Ability to work weekends, evenings and flexible hours, available to peak service delivery days and times

Responsabilités

• Identifies on-going needs and services of residents through the assessment/PSP process
• Ensures proper follow through and documentation for residents with changes in clinical care needs
• Supports the Wellness Nurse with completing the clinical sections of monthly wellness visits and PSP
• Informs the Wellness Director and/or Executive Director of changes in medication or service level
• Assist with obtaining weights and vital signs monthly for each resident prior to monthly wellness visit
• Assist with resident care as needed
