• Assist clients with daily living activities such as bathing, dressing, grooming, toileting and mobility.
• Provide companionship and emotional support.
• Perform light housekeeping, meal preparation, and medication reminders.
• Follow the care plan provided by the nurse and report changes promptly.
• Maintain accurate, timely documentation of visits and services.
• Coordinate with office staff as needed.

Responsibilities:

• Driver’s license and reliable transportation preferred

• Must pass background check and health screening
