Job summary

Assisted Hospice is seeking a per diem Certified Home Health Aide (CHHA) for our Los Angeles/Culver City branch. We have about 10-15 visits per week, with rates of $29 per visit plus $11 for mileage. Qualifications

• Completion of an approved training program pursuant to Federal/State law.
• Current CHHAcertificate with criminal background check (California).
• Current CPR, health clearance and completed skills assessment.

Responsibilities

• Assist patient with personal hygiene, elimination, nutritional support, ambulation and exercise as per CHHA assignment.
• Ensure quality and safe delivery of CHHA services per plan of care.
• Communicate with RN or Clinical Supervisor on changes in patient condition.

The impact you will make:

• Assist patient with personal hygiene, elimination, / nutritional support, ambulation and exercise according to the patient’s needs and as specified on the CHHA Assignment.
• Ensure quality and safe delivery of CHHA services per written plan of care.
• Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient’s condition and/or needs.
