Bristol Hospice Visalia is seeking a Certified Home Health Aide (CHHA) to join our compassionate team in Visalia, CA. Under RN supervision, you will provide personal care, assist with daily living activities, and support patients and families in a hospice setting.

Ideal candidates hold a valid CNA with CA HHA certification, possess CPR, and have a caring, patient-centered approach. Training and a supportive team environment are provided to ensure high-quality care.

Qualifications

• Ability to read and follow written instructions
• Comfortable providing specialized hospice care
• Ability to work autonomously with minimal supervision
• Strong communication and interpersonal skills
• Excellent organizational skills and attention to detail
• Familiarity with plan of care documentation

Responsibilities

• Perform personal care tasks and attend to patient requests
• Provide compassionate communication with patients and families
• Ensure safety and proper use of equipment
• Assist with bathing, grooming, and linen changes
• Assist with dressing, meals, and feeding
• Record vital signs and report changes to Case Manager
• Support mobility and transfers per care plan
• Assist with medication reminders as allowed by state rules
• Maintain clean, orderly patient living area
• Adhere to care procedures and professional conduct
