Job summary

Responsibilities

• Provide companionship activities and homemaking assistance.
• Assist with personal care, grooming and mobility needs.
• Take vital signs and monitor health status as directed.
• Support post-stroke and dementia care tasks as assigned.
• Assist with light housekeeping, laundry and meal preparation.
• Transport to appointments and adhere to patient care plans.

• transportationto appointments
• adherence to patient plan of care

Minimum Education & Experience Requirements:

• New Jersey Certified Homemaker Home Health Aide Certificate
• Reliable vehicle
• Ableto pass criminal history and drug screening
• Authorized to work in the United States
• Provide two references of past employers or nursing program instructors (no family or friends)
• Able to constantly travel within geographic area serviced by office from assignment to assignment.
