Job summary

CVHCare is seeking a Certified Home Health Aide (CHHA) to join our home health care team in the San Ramon area. You will work under supervision to provide skilled services and assist patients with daily activities, monitoring vitals and documenting care in EMR.

The role requires CA CHHA certification and CPR, with strong attention to detail, reliability, and excellent communication. Part-time or Per Diem schedules are available, and compensation is based on a full-time schedule.

Qualifications

• Minimum one year experience as CHHA; home health care setting preferred.
• CA CHHA certification and CPR certification are required.
• Understanding of clinical terminology and charting practices.
• Proficient with computers; able to use MS Word/Excel/Outlook and email.
• Excellent written and verbal communication; strong time management.

Responsibilities

• Provide skilled services and assist with daily activities under clinician supervision.
• Check vitals and document findings in patient charts.
• Assist with exercises and activities per clinician plan.
• Support with transfers, grooming, feeding, and light housekeeping.
• Utilize computer systems and EMR for accurate documentation.

About the position:

Our Certified Home Health Aides (CHHA) work in collaboration and under the supervision of a Licensed/Certified Clinician. This role is responsible for providing patients with skilled services and assistance with basic daily activities.

As a Certified Home Health Aide (CHHA), you will use your education to assist with activities which support skilled services including checking temperature, pulse and respiration rates, assisting with exercises, or repetitive routines as prescribed by the clinicians. The nurturer you are, will put your skills to work in support of other daily activities that may include assistance with getting in and out of bed, dressing, grooming, hygiene, feeding, assistance with ambulatory transfers, and light housekeeping.

Schedule:

Part Time 24 = 18 RVEs p/week or Per Diem (12 visits p/week)
* Non-traditional schedules are available, i.e., Thursday-Monday

Locations:

Area #6 This territory includes, but is not limited to, the cities of Vacaville, Fairfield, Suisun City, Rio Vista, Dixon

Area #7 covers the following cities - Vallejo, Benicia, Napa, American Canyon, Yountville

Required Qualifications:

• Minimum one (1) year experience working as a Certified Home Health Aide (CHHA)
• Minimum one (1) year checking vitals and reporting/documenting on clinical charts
• Home Health Aide Training
• California Home Health Aide Certification and CPR Certification
• General knowledge and understanding of clinical verbiage
• Strong attention to detail for use in chart documentation
• Excellent time management/calendaring skills and ability to deliver documentation timely
• Strong experience using computers and computer systems
• Experience using Microsoft Suite (Word, Excel, Outlook), Gmail Suite or other systems
• Excellent written and verbal communication

Compensation is based on full time schedule
