Job summary

Springpoint Senior Living, Inc. in Monroe Township, NJ, is seeking Certified Home Health Aides (CHHAs) for per diem positions to provide private duty home care and assist clients with daily living activities.

You will follow care plans, monitor health, assist with bathing and meals, and offer companionship under licensed staff supervision. A valid NJ CHHA license and a high school diploma are required, with competitive per diem pay.

Qualifications

• Per diem schedule and private duty home care experience preferred.

Responsibilities

• Provide ADL support per care plan.
• Monitor patient health and document changes daily.
• Assist with bathing, grooming, dressing, and meals.
• Support transfers and mobility as needed.
• Offer companionship and emotional support under supervision.
• Maintain clean living spaces and report observations.
• Transport or accompany to activities when scheduled.

Description

Springpoint at Home is seeking Certified Home Health Aides (CHHAs) to join our team!

Springpoint at Home provides private duty home care services to our clients. We are seeking Per Diem CHHAs to join our team! At Springpoint, our CHHAs are responsible for providing care and assistance to the patients with activities of daily living in accordance with the patients care plan. The CHHA enables patients to stay in their residence by monitoring and recording patient conditions and providing support and companionship under the direction of licensed professional staff.

Responsibilities:

• Observe the patient’s general health conditions, progress, or any unusual health issues and maintain daily records to share with family or supervisor.
• Guides patients in simple prescribed exercises per patient care plan.
• Assist patients with transferring when necessary, including but not limited to in and out of beds, baths, wheelchairs.
• Assist with bathing, skin care, grooming, dressing, and undressing.
• Maintain records of patient care, condition, progress, and problems to report and discuss observations with supervisor.
• Change bed linens and clean patient’s quarters
• Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert. Provide emotional support as needed.
• Serve meals to patients according to prescribed diets and patient request. Clean up after meals in the home as needed.
• Encourage, transport, or accompany patient to recreational activity programs.

Education and Experience:

• High school diploma or equivalent required
• Valid NJ CHHA License
