Job summary

Springpoint at Home is seeking Certified Home Health Aides (CHHAs) to join our team for per diem shifts in the Monroe Township area. The CHHA will provide personal care and support to patients at home per the care plan, monitor conditions, and deliver companionship under licensed supervision.

Responsibilities include observing health status, assisting with transfers, bathing, meals, and daily activities, while keeping records and reporting to supervisors.

Qualifications

• High school diploma or equivalent required.
• Valid NJ CHHA License required.

Responsibilities

• Observe the patient’s general health conditions, progress, or any unusual health issues and maintain daily records to share with family or supervisor.
• Guide patients in simple prescribed exercises per patient care plan.
• Assist patients with transferring when necessary, including but not limited to in and out of beds, baths, wheelchairs.
• Assist with bathing, skin care, grooming, dressing, and undressing.
• Maintain records of patient care, condition, progress, and problems to report and discuss observations with supervisor.
• Change bed linens and clean patient’s quarters.
• Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert. Provide emotional support as needed.
• Serve meals to patients according to prescribed diets and patient request. Clean up after meals in the home as needed.
• Encourage, transport, or accompany patient to recreational activity programs.

Springpoint at Home is seeking Certified Home Health Aides (CHHAs) to join our team!

Springpoint at Home provides private duty home care services to our clients. We are seeking Per Diem CHHAs to join our team! At Springpoint, our CHHAs are responsible for providing care and assistance to the patients with activities of daily living in accordance with the patients care plan. The CHHA enables patients to stay in their residence by monitoring and recording patient conditions and providing support and companionship under the direction of licensed professional staff.

Responsibilities:
