Description Minimum Qualifications:
* High School diploma or equivalent required.
* Successful completion of a home health aide training program meeting national standards required.
* Home Health Aide certification in the State of New Jersey required.
* BLS certification required within 30 days from the date of hire or transfer.
* Valid NJ driver's license, registration, and insurance required.
* Safe driving record also required.
* Minimum 1 year of experience working with the frail or elderly.
Position Summary:
The PACE LIFE Program Home Health Aide provides assistance with activities of daily living, restorative, and supportive care to preserve the home environment. This role reports health status and ensures a safe environment for participants. The aide ensures care aligns with the treatment plan established by the primary nurse and adheres to other standards. The position supports organizational goals through quality customer service, participation in performance improvement, and teamwork.
Qualifications:
- Education: High School diploma or equivalent; completion of a nationally recognized home health aide training program.
- License/Certification: Home Health Aide certification in New Jersey; BLS certification within 30 days of hire; valid NJ driver's license, registration, and insurance; safe driving record.
- Experience: At least 1 year working with the elderly or frail individuals. Proficiency in clinical applications is preferred at hire; training will be provided, and skill is expected within 30-60 days.
Performance Expectations:
Demonstrate technical competencies as outlined in the Assessment and Evaluation Tool.
Work Environment:
Work effectively within an interdisciplinary team in an Adult Day Health Center and primary care clinic setting. Must have normal vision and hearing, manual dexterity, and the ability to lift up to 30 pounds using proper body mechanics. Communicate effectively with staff, vendors, and clients.
Reporting Relationship:
Reports to department leadership.
