Job summary

Maxim Healthcare is hiring to support patients at home, providing essential in-home care according to the plan of care. The role involves assisting with activities of daily living, safeguarding patient safety, and documenting health observations for the supervising nurse.

Qualifications

Responsibilities

• Assist patients with plan of care tasks such as baths, oral hygiene, dressing, toileting, and feeding
• Record vital signs when ordered and ensure competency demonstrated
• Assist with safe transfers and ambulation
• Carry out exercises per plan of care
• Perform simple extensional therapies as ordered with competency
• Handle patient laundry and maintain safety using equipment correctly
• Observe and teach home safety practices and emergency procedures
• Communicate clearly with client, family, and office; report changes to nurse supervisor

Responsibilities:

• Responsible for assisting patient with and per plan of care, the following;
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements
