Job Description

This position is responsible for providing the clinical educationto patients with diabetes on self-management skills. The individualwill organize and oversee various areas of patient diabetesmanagement, to include, but not limited to education, compliance,and follow-up coordinating care among multi-disciplinary team ofhealthcare providers.

Minimum Qualifications

• Bachelor's degree required

• Certified diabetic educator required
• Must have clinical experience in a hospital or clinic setting orcan be a qualified health care professional
• Excellent communication skills required
