Bryan Health in Lincoln, NE seeks an expert Diabetes Education Specialist to provide routine and complex diabetes education across all ages. You will manage a diabetes caseload, collaborate with physicians, and drive education programs aligned with ADA standards and national guidelines.

The role emphasizes evidence-based interventions, ongoing evaluation, and senior-level communication with the care team to improve glycemic outcomes and patient self-management skills.

Qualifications

• Current Nebraska RN licensure or eligible compact state licensure.
• Minimum of two years of clinical practice.
• Certified Diabetes Educator certification preferred.
• For RD path: RD with LMNT licensure and CDR registration preferred.

Responsibilities

• Commit to mission, vision, beliefs and core values.
• Assess patient needs and tailor education plans based on learning barriers and goals.
• Assess patient/family knowledge and document actions per Bryan policy.
• Incorporate cultural considerations into care planning.
• Deliver diabetes education per ADA standards and 15 content areas.
• Maintain up-to-date diabetes care knowledge and self-management education.
• Collaborate with interdisciplinary team to meet patient and program goals.
• Identify barriers to care and contribute to solutions.
• Develop strategic plans for glycemic management and program evaluation.
• Coach clinical staff in education practices related to glycemic management.
• Act as a consultant to ensure continuity of care and best practices.
