Job summary

Bayhealth Medical Center is seeking a Certified Coder to abstract medical records and assign ICD-9/ICD-10 and CPT-4 codes for optimal hospital reimbursement. The role requires working with DRG grouping and coding databases to ensure accurate data and medical necessity.

Qualifications include a CPC or RHIA credential with at least six months of coding experience; education includes a Medical Coding & Billing certificate and a high school diploma or GED.

Qualifications

• Education: Certificate Program High School Diploma or GED; Medical Coding & Billing.
• Credential: CPC or RHIA (CPC-A acceptable).
• Six months to one year coding/medical records experience preferred.

Responsibilities

• Codes data from patient records using ICD-9/ICD-10 and CPT-4 codes.
• Abstracts data to compile statistics with the DRG grouper.
• Verify medical necessity and correct codes with coding databases.
• Responds to physician inquiries regarding coding and documentation.
• All other duties as assigned within coding scope.

SALARY RANGE: 23.32 - 34.97 HOURLY

General Summary:

Abstracts clinical information from a variety of medical records, charts and documents and assigns appropriate ICD-9 /ICD-10 and/or CPT-4 codes to patient records according to established procedures. Works with coding databases and confirms DRG assignments.

Responsibilities:

1. Codes data from patient records utilizing computerized coding systems to ensure optimal reimbursement of the hospital. Translates diagnostic and procedural information into the International Classification of Diseases codes and sequences these codes in a manner which legitimately optimizes hospital reimbursement through the use of a computerized encoding and DRG grouper application. 2. Abstracts data from patients' medical records using the computerized abstracting system in order to compile accurate and timely statistical data. The Coder reviews and analyzes all patient types to determine the principal diagnosis and procedure and other significant diagnoses and procedures, especially for those which reimbursement is DRG dependent. 3. Identifies and verifies medical and surgical complications and comorbidities as well as medical necessity. Works independently reviewing medical record documentation, laboratory reports, radiology reports, and consultation reports to identify medical conditions evaluated and treated during the visit. 4. Corrects abstracts with incorrect codes requested by Finance or third party payors to ensure compliance with required regulations and accreditation standards. 5. As needed, responds to physician inquiries that concern the proper documentation of diagnostic procedural information and questions regarding coding assignments. 6. All other duties as assigned, within the scope and range of job responsibilities.

Required Education, Credential(s) and Experience:

• Education: Certificate Program High School Diploma or GED; Medical Coding & Billing;
• Credential(s): Certified Professional Coder; Or: CPC-A, RHIA
• Experience: Required: Six months experience in coding or medical records using ICD and CPT coding systems. Preferred: One year coding experience in healthcare facility.
