Responsibilities

• Coordinate group and individual activities for patients, considering interests and needs.
• Collaborate with volunteers, families, and community agencies.
• Document observations, interventions and outcomes for residents.
• Create a home-like atmosphere and community involvement for patients.
• Plan and facilitate individual and group activities.
• Maintain records and participate in care plan meetings.
• Develop, update and distribute activity-related policies and procedures.
