About the Role: Job SummaryThe scheduler is the Team Leader for the aide staff. This position is critical for scheduling clinical staff to adequately meet the needs of patient census and responsible for accurately maintaining the master schedule for designated clinical staff. Works collaboratively with the IDT members and Patient Care Manager (PCM) to provide coordination of patient care to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Performs chart audits under the direction of the PCM in timely manner to ensure efficient and effective patient care. Serves as a link between the PCM, nursing staff, billing, intake and patient and family to ensure quality patient care and perform phone surveys to ensure satisfaction of hospice services. Utilizes knowledge of patient's age, gender and cultural diversity into the provision of patient care. Contribute to ensuring the provision of quality nursing care through performance improvement processes that demonstrate positive outcomes in patient care.
Minimum Job QualificationsLicensure or other certifications:
Certified Nursing Assistant who has successfully completed a training and competency evaluation program that meets federal and state regulations. BCLS certification.
Educational Requirements: High school diploma and
Medical terminology required.
Minimum Experience:
3-5 years of clinical licensure experience, preferably in the hospice, or home health environment.

1. Responsible for accurately maintaining the master schedule for designated clinical staff.
1A. Responsible for arranging coverage for clinical staffing needs.
1B. Responsible for updates to clinical master schedule on a daily basis.
1C. Must be able to communicate effectively with clinical staff regarding scheduling for coverage of patient care needs. Notifies clinical staff as appropriate of changes in patients' condition/support system/environment.
1D. Must be able to communicate effectively with supervisor and provide updates of scheduling status daily.
2. Responsible for the designated hospice clinical schedule for direct patient care needs.
2A. Plans daily and weekly schedule for hospice clinical staff with assigned visit frequency per plan of care and efficiency of staff schedule.
2B. Makes changes to schedule accordingly as care plans are updated, frequency changes or through communications by RN on daily basis.
2C. Coordinates communication of patient care needs and changes to hospice clinical staff verbally and through electronic documentation system.
2D. Handles all calls with excellent customer service, compassion, and interpersonal expertise to patients/families and staff.
2E. Report directly to the Patient Care Manager regarding documentation issues and any changes that must be made to the plan of care to meet these requirements.
3. Facilitates the efficient operations of the service.
3A. Attends and leads daily morning huddles and end-of-week afternoon huddle.
3B. Attends staff/team meetings as required by the Patient Care Manager.
3C. Documents and processes variances and other reports as assigned in accordance with hospice policy and procedure.
3D. Participates in review of clinical audits, peer review and other QAPI related activities as directed.
3E. Prepares standard reports and assists manager in reviewing reports such as productivity, overtime, visit frequencies, mileage, etc.
3F. Completes accurate daily log with efficiency and minimum overtime.
3G. Serves as a source of reference along with the staff education supervisor to answer questions and provides ongoing patient care and procedural support for the Aide staff.
4. Maintain responsibility for communication and professional development and growth.
4A. Attends interdisciplinary team meetings, staff meetings and educational programs when indicated and/or obtains information through review of minutes, handout materials, video/audio tapes of sessions.
4B. Completes all required education within specified time frames.
4C. Observes established dress guidelines and visibly wearing photo/name identification badge.
4D. Demonstrates good judgment and critical thinking skills in response to calls. Inspires confidence from callers and co-workers, remaining well informed and prepared at all times. Demonstrates caring and maintains calm and effective behavior during stressful situations.
5. Reports problems/concerns regarding patients to RN/Patient Care Manager.

8. Performs other job related duties or functions as requested or assigned. Willingly accepts assignments to special projects.

skills competency; supports department-based goals which contribute to the success of the organization; serves as
preceptor, mentor, and resource to less experienced staff.
11. Assists with orientation and every day workflow of assignments of new employees. Assists with development of in-services
for staff members. Coordinates daily workflow with staff members to ensure timely completion.
12. Assists in the supervision, coaching, mentoring, leading and training of department personnel.
