Job summary

Responsibilities

• Maintains a Referral Data Base and daily referral list.
• Tracks referrals and documents appointments kept or missed.
• Assists with insurance forms prior to appointments.
• Faxes referral forms to facilities and coordinates preparation.
• Supports a team-based approach to patient-centered care.

ESSENTIAL JOB FUNCTIONS

• Performs tasks pertinent to age of patient (infancy to geriatric).
• At the written order from a CVHS Provider, makes outside referral appointments.
• Serves as a liaison between providers and patients for outside referrals.
• Maintains a Referral Data Base.
• Prints out a referral list each day.
• Follows a tracking system by following up on all referrals. Provides written documentation of appointments kept or not kept.
• Receives written or verbal documentation from providers for missed appointments.
• Provides assistance with Insurance forms as needed prior to making appointments.
• Faxes completed referral forms to appropriate facility.
• Coordinates with the provider and pharmacy, appointments requiring a special preparation.
• Provides special instruction sheets as needed.
• Supports a team based approach to patient-centered medical care.
• Attends work as scheduled.
• Performs other duties as assigned.
