Avalon Health Care Management, Inc. in Salt Lake City, UT is seeking a Central MDS Coordinator (RN) to lead the MDS and resident assessment process across facilities.

The role requires an active RN license and prior MDS/facility experience to ensure timely, accurate documentation and reimbursement compliance. You will coordinate care planning with an interdisciplinary team and support Medicare/Managed Care requirements, driving quality outcomes across multiple markets.

Qualifications

• Active RN license in good standing.
• Experience with MDS and skilled nursing facilities required.
• Strong knowledge of the RAI process and case management.

• Ensure assessments and documentation are completed accurately and on time.
