Location Detail: 765 Fairfield Ave Bridgeport (10411)

Shift Detail: Monday through Friday 8a-4:30p, with some weekend requirements and 1 major and 1 minor holiday per year.

With a goal of achieving excellence in every patient and customer experience, the core purpose of the Central Intake Coordinator (CIC) is to review referral documentation, assess and align appropriate care levels, and coordinate services based on the patient's goals of care. Responsibilities include ensuring referral record completeness, following up on missing documents, communicating with care teams, and managing patient transfer and admission data.

The CIC will:

  1. Become educated on healthcare levels and facilitate timely care transitions, serving as a bridge between the healthcare team, patients, and caregivers.
  2. Coordinate and communicate referrals and patient care with onsite transitional care coordinators and clinical areas for seamless care delivery.
  3. Locate transferred patients, communicate with clinical teams, and resolve inquiries related to referral data for coding and billing.
  4. Maintain effective communication, demonstrate self-direction, teamwork, curiosity, ownership, flexibility, and model leadership behaviors.
  5. Ensure work accuracy and adherence to regulatory and organizational policies.
  6. Support team performance and professional development needs.
  7. Collaborate with healthcare providers to ensure continuity of care, obtain verbal orders, and coordinate orders and follow-up appointments.
  8. Review demographic and clinical documentation for accuracy and regulatory compliance, ensuring proper transition processes.
  9. Maintain communication with management to resolve issues and ensure satisfaction of patients and referral sources.
  10. Provide community resource consultations and support positive relationships across the continuum of care.
  11. Utilize clinical judgment to identify risks and respond appropriately.
  12. Act as a liaison internally and externally, ensuring timely follow-up on transfer patients.
  13. Assist with face-to-face requirements and MD verification during high work volumes.
  14. Maintain confidentiality according to HIPAA and other regulations.
  15. Demonstrate leadership behaviors and support team culture and initiatives.

Qualifications

Education: LPN with an active license in CT; Bachelor's degree preferred.

Minimum Experience: At least three years of nursing experience in a clinical specialty area.

Preferred Experience: At least two years in homecare.

Language Skills: Strong written and verbal communication skills.

Knowledge, Skills, and Abilities: Effective communication across all organizational levels, project management, problem-solving, industry standards knowledge, and proficiency in MS Office and Project.
