The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with laws, regulations, and agency policies. Responsibilities include:

• Coordinating care plans with healthcare professionals and ensuring continuity of care through effective communication with physicians, patients, families, and community resources.
• Conducting initial and comprehensive nursing evaluations, developing care plans within state guidelines, and ensuring accurate documentation.
• Performing patient assessments and developing appropriate care strategies.
• Ensuring patients meet home health eligibility and medical necessity guidelines.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation

Additional State Requirements (Louisiana):

• Minimum one year of clinical experience as an RN; experience as an LPN may be considered in lieu, provided RN licensure is unrestricted.
