Job summary

UnitedHealth Group's Caretenders, part of the LHC Group and within the Optum family, seeks a Central Intake Coordinator in Home Health for an office-based, full-time role with a comprehensive benefits package.

The coordinator triages incoming calls, collects patient and referral information, and coordinates with Central Intake Staff and branches to ensure accurate, timely referrals. LPN licensure and healthcare experience are required.

Qualifications

• LPN licensure in the state of practice.
• 1+ years experience in office procedures.
• 1+ years Insurance/Medical experience.

Responsibilities

• Answers phones and appropriately triages phone calls to Central Intake Teams.
• Effectively communicates patient/referral information to Central Intake Staff and branches.
• Obtains demographic and clinical information from clients and physicians to compile referrals.
• Coordinates communication of referral information with Central Intake Nurses.
• Transfers High Tech/nurse specific referral calls to Central Intake Nurses.
• Triages incoming calls and messages from referral sources and relays messages as appropriate.
