Job summary

White Plains Hospital Medical Center is seeking a Clinical Documentation Improvement Specialist (CDIS) to ensure accurate documentation for severity of illness and quality in the medical record. You will review records, collaborate with physicians, and educate staff on the CDIP process.

The role requires a BSN or PA degree and 4–6 years of adult acute care experience, with strong analytical and writing skills. This is a hybrid remote position based in White Plains, NY with day shifts.

Qualifications

• BSN or Physician Assistant degree required.
• 4–6 years of adult acute care experience in med/surg, critical care, ED, or PACU.
• Experience with official coding guidelines and documentation requirements for inpatient payment systems.

Responsibilities

• Understand and adhere to performance standards, policies and behaviors.
• Review medical records for completeness and accuracy of SOI/quality.
• Conduct initial and concurrent record reviews in CDI software and EMR.
• Identify opportunities for documentation improvement and clarification queries.
• Facilitate documentation to support accurate diagnosis coding and DRG assignment.
• Draft clinically credible clarification queries to improve documentation and comorbidities.
• Request documentation clarifications for principal diagnosis, SOI, mortality risk, and quality measures.
• Communicate with physicians, NPs, case managers, coders and care team members.
• Follow up on all cases and resolve clarifications.
• Collaborate with HIM coding staff to resolve discrepancies.
