Job summary

Metropolitan Family Services in Merrionette Park is seeking a CCBHC Evaluation & Treatment Planning Specialist to perform comprehensive assessments, document diagnoses, and craft strengths-based, IM+CANS–driven treatment plans. You will apply ASAM Criteria across six dimensions, collaborate with a multidisciplinary team, provide crisis intervention, case management, and ensure timely, compliant records.

Qualifications include a Master’s degree in a behavioral science field and licensure

Qualifications

• Master's degree in a behavioral science field is required.
• Eligible for LSW/LPC licensure in the state.
• Must obtain IM+CANS certification.
• Training in ASAM Criteria within six months of employment is required.
• 2+ years of post-graduate social work experience preferred.
• CADC or substance use experience preferred.
• IL LCPC/LCSW preferred; IM+CANS certification preferred.

Responsibilities

• Perform comprehensive assessments and clinical formulation with accurate diagnosis and strengths-based treatment plans.
• Use ASAM Criteria to evaluate clients across six dimensions.
• Develop initial and updated treatment plans with client and support system.
• Provide crisis intervention and case management services.
• Collaborate with multidisciplinary team to ensure seamless care and warm handoffs.
• Maintain timely, accurate client records and required reporting.
• Participate in trainings and meetings; work with diverse populations.

Education

Master's degree in Social Work, Counseling, Psychology, or related behavioral science field

Licensed Social Worker (LSW) / Licensed Professional Counselor (LPC) eligibility

SALARY:

The average starting salary for this position will fall in the range of$64,000and$72,000annually.Final salary will be determined based on years of experience, licensure level, and relevant credentials
$2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment).

ESSENTIAL JOB FUNCTIONS:

• Performs comprehensive, in-depth assessment and clinical formulation, development ofaccuratediagnosis andstrengths-based treatment plans for CCBHC services. Utilize the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) to document clinical needs and strengths.
• UsesASAM Criteria toevaluate clients across six specific dimensions, including withdrawal potential, biomedical conditions, and recovery environment.
• Facilitates the development ofinitialand updated treatment plans in collaboration with the client and their support system.
• Usesstandardized criteria (e.g., ASAM for addiction) todeterminetheappropriate intensityof services.
• Performsopen accessas assigned.
• Collaborateswith a multidisciplinaryteam,including Peer Support Specialists and medical staffto ensure seamless transitions and "warm handoffs".
• Provides crisis intervention, and case management services, adhering to CCBHC and MRO/Medicaid rehabilitation service models.
• Provides therapeutic interventions, advocacy, skill-building, and related services.
• Provides evidence-based treatment interventions per training and certifications.
• Learnsandutilizesagency and community resources.
• Assesses community conditions that affect clients toidentifyresource gaps and problems.
• Works with multidisciplinary professionalsincluding psychiatrists,nursesand community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
• May serve on regional or agency-wide committees.
• Meets agency expectation for quantity of direct serviceandmaintainsquality and fidelity to evidence-based practices.
• Maintainstimelyandaccurate client records and any necessary agency and CCBHCrequiredreporting.
• Participates in agency and sitebasedrequired meetings andtrainings.
• Works effectively with diverse staff and service population.
• Other duties as assigned.

KNOWLEDGE, SKILLS, and ABILITIES:

• Demonstrated superior skills with families or communities.
• Demonstrated competence in consultation, teaching, or writing.
• Requires mobility to work in the community with assigned caseload.
• Ability to work effectively with diverse individuals and groups.

QUALIFICATIONS:

• Master's Degree in Social Work, Counseling, Psychology, or a related behavioral science field. from an accredited college or university,required.
• Eligible or a Licensed Social Worker (LSW), Licensed Professional Counselor (LPC),required.
• Must obtain and maintain IM+CANS certification,required.
• Must attend at least one training relative to the application of ASAM Criteria within the first six months of employment,required.
• Final salary will be determined based on years of experience, licensure level, and relevant credentials,required.
• 2+ years of post-graduate social work experience at an intermediate skill level,preferred.
• Certified Alcohol and Drug Counselor (CADC) or experience working with clients experiencing substance use,preferred.
• Experiencewith ASAMCriteria,preferred.
• Licensed Clinical Professional Counselor(LCPC) orLicensed Clinical Social Worker (LCSW)in the State of Illinois,preferred.
• IM+CANS certification,preferred.

ADDITIONAL REQUIREMENTS:

• Satisfactorily pass all relevant screening and criminal background checks.

TRANSPORTATION/TRAVEL REQUIREMENTS:

• Driving for workrequiredwith transportation of clients preferred in a personally owned vehicle.No travel between sites
