Job summary

Metropolitan Family Services in Merrionette Park, IL, seeks a CCBHC Evaluation & Treatment Planning Specialist for on-site, full-time work. You will conduct comprehensive assessments, formulate diagnoses, and develop strengths-based treatment plans for clients using IM+CANS and ASAM Criteria, collaborating with a multidisciplinary team.

You will provide crisis intervention, case management, and evidence-based interventions while maintaining accurate records and participating in trainings.

Qualifications

  • Master's degree in a behavioral field; required.
  • Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC); required.
  • IM+CANS certification; required.
  • Must complete ASAM Criteria training within the first six months; required.
  • 2+ years post-graduate experience; preferred.
  • CADC certification or experience with substance use; preferred.
  • IL licensure as LCPC or LCSW; preferred.
  • IM+CANS certification; preferred.

We are currently looking to fill an Evaluation & Treatment Planning Specialist role at our Merrionette Park Office.

SALARY:

  • The average starting salary for this position will fall in the range of $64,000 and $72,000 annually.

$2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment).

ESSENTIAL JOB FUNCTIONS:

  • Performs comprehensive, in-depth assessment and clinical formulation, development ofaccuratediagnosis andstrengths-based treatment plans for CCBHC services.
    • Utilize the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) to document clinical needs and strengths.
    • UsesASAM Criteria toevaluate clients across six specific dimensions, including withdrawal potential, biomedical conditions, and recovery environment.
    • Facilitatesthe development ofinitialand updated treatment plans in collaboration with the client and their support system.
    • Usesstandardized criteria (e.g., ASAM for addiction) todeterminetheappropriate intensityof services.
  • Performsopen accessas assigned.
  • Collaborateswith a multidisciplinaryteam,including Peer Support Specialists and medical staffto ensure seamless transitions and "warm handoffs".
  • Provides crisis intervention, and case management services, adhering to CCBHC and MRO/Medicaid rehabilitation service models.
  • Provides therapeutic interventions, advocacy, skill-building, and related services.
  • Provides evidence-based treatment interventions per training and certifications.
  • Learnsandutilizesagency and community resources.
  • Assessse community conditions that affect clients toidentifyresource gaps and problems.
  • Works with multidisciplinary professionalsincluding psychiatrists,nursesand community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
  • May serve on regional or agency-wide committees.
  • Meets agency expectation for quantity of direct service andmaintainsquality and fidelity to evidence-based practices.
  • Maintainstimelyandaccurateclient records and any necessary agency and CCBHCrequiredreporting.
  • Participates in agency and sitebasedrequired meetings andtrainings.
  • Works effectively with diverse staff and service population.
  • Other duties as assigned.

KNOWLEDGE, SKILLS, and ABILITIES:

  • Demonstrated superior skills with families or communities.
  • Demonstrated competence in consultation, teaching, or writing.
  • Requires mobility to work in the community with assigned caseload.
  • Ability to work effectively with diverse individuals and groups.

QUALIFICATIONS:

  • Master's Degree in Social Work, Counseling, Psychology, or a related behavioral science field. from an accredited college or university, required.
  • Eligible or a Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), required.
  • Must obtain and maintain IM+CANS certification, required.
  • Must attend at least one training relative to the application of ASAM Criteria within the first six months of employment, required.
  • Final salary will be determined based on years of experience, licensure level, and relevant credentials, required.
  • 2+ years of post-graduate social work experience at an intermediate skill level, preferred.
  • Certified Alcohol and Drug Counselor (CADC) or experience working with clients experiencing substance use, preferred.
  • Experiencewith ASAMCriteria , preferred.
  • Licensed Clinical Professional Counselor(LCPC) orLicensed Clinical Social Worker (LCSW)in the State of Illinois , preferred.
  • IM+CANS certification , preferred.

ADDITIONAL REQUIREMENTS:

  • Satisfactorily pass all relevant screening and criminal background checks.

TRANSPORTATION/TRAVEL REQUIREMENTS:

  • Driving for work required with transportation of clients preferred in a personally owned vehicle . No travel between sites

PHYSICAL DEMANDS:

While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
