What does a Transitions at Home CBRF RN Consultant/ RN Home Health Case Manager do?
Transitions At Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and assistance with audits and policy development. Ideal for an RN who has strong knowledge of assisted living regulations. Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours:

  • Full-Time
  • 40 hours per week 8:00 am - 4:30pm
  • Monday through Friday
  • No Night Shifts
  • Limited on-call rotation

Qualifications / Job Requirements:

  • Active Wisconsin Registered Nurse License
  • Two (2) years’ nursing experience, 1 year in Home Health (preferred)
  • CPR Certification
  • Wound Care would be beneficial
  • A valid Wisconsin Driver's License and Car Insurance
