We are seeking an experienced CBRF RN Consultant / RN Home Health Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care.

What does a Transitions At Home CBRF RN Consultant / RN Home Health Case Manager do?

The RN will provide expert guidance and oversight in clinical and regulatory matters. Responsibilities include staff training, care plan review, incident investigation, assisting with audits and policy development. The role is ideal for an RN with strong knowledge of assisted living regulations, coordinating services, and counseling patients and families/significant others in their residence, in accordance with physician orders and the plan of care, under the supervision of the RN Clinical Manager.

Hours:

• Full-Time
• 40 hours per week, 8:00 am - 4:30 pm
• Monday through Friday
• No Night Shifts
• Limited on-call rotation

Qualifications / Job Requirements:

• Active Wisconsin Registered Nurse License
• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• Wound Care experience is beneficial
• A valid Wisconsin Driver's License and Car Insurance
