Job summary

Wellington Place at Biron is seeking an experienced Facility Director to lead our 30-bed CBRF in Wisconsin Rapids. This on-site role directs daily operations, guides a compassionate team, and champions resident-centered care with emphasis on quality and service.

You will ensure regulatory compliance, mentor staff, and collaborate with the Care Coordinator to improve resident outcomes. A hands-on leader with prior geriatrics or long-term care experience is preferred.

Qualifications

• Must be at least 21 years old.
• Demonstrate leadership and supervisory skills.
• Must speak and understand English.
• Previous experience in healthcare, geriatrics, or a long-term care setting.
• Patience, tact, enthusiasm and positive attitude toward the elderly.

Responsibilities

• Oversee and manage the CBRF.
• Serve as a role model and mentor to all employees.
• Ensure compliance with DHS CH 83 regulations.
• Be responsible for residents, staff and building 24 hours.
• Report to VP of Operations at Carriage Healthcare Companies, Inc.
• Collaborate with Care Coordinator on clinical resident outcomes.
• Conduct resident referral assessments and involve Care Coordinator as needed.
• Oversee admission paperwork, assessments, medication review, and ISP implementation.

Job Summary:
As a Facility Director at Wellington Place at Brion, you will oversee daily operations, inspire and develop a strong team, and foster a welcoming, resident-centered environment where quality care and exceptional service are top priorities.

Compensation:

• $48,000.00 - $55,000.00 annually, dependent on experience

Key Responsibilities:

• Oversee and management of the CBRF.
• Serve as a role model and mentor to all employees.
• DHS CH 83 Regulations comprehension and compliance.
• Responsible for residents, staff and building 24 hours.
• Works under and reports directly to Vice President of Operations, Carriage Healthcare Companies, Inc.
• Work closely with Care Coordinator for clinical resident outcomes.
• Resident referral assessments. Include Care Coordinator when appropriate.
• Upon resident admission: assess, initiate and complete all admission paperwork, complete assessments, i.e., fall/fall interventions/wanderer/Braden scale/evacuation, review and verify medications, treatments, and ADL’s in E-Mar, implement initial ISP.
• Monitor staff progress notes for timely and accurate documentation.

Qualifications:

Required:

• Must be at least 21 years of age
• Demonstrate leadership and supervisory skills
• Must speak and understand English
• Previous experience in healthcare, geriatrics, or a long-term care setting
• Patience, tact, enthusiasm and positive attitude toward the elderly
• High school diploma

Preferred Qualifications:

• Leadership/Management experience in a healthcare or business setting
• Education/Certification in healthcare
• Experience working as a Certified Nursing Assistant, Resident Care Assistant, or Licensed Practical Nurse
• Qualifications under DHS Chapter 83.15 specifically (d)
